How it is delivered

Remote first, written always. You receive a scope note before work starts and a written output at the end. Nothing consequential leaves without a named person signing it.

When something doesn’t feel right, see the whole picture.

From a support worker on a late shift to a GP, a paramedic, a social worker, a registered manager, a regional director or the person who owns the company, ORVIA helps the right information reach the right person with the evidence and the accountability still attached. Do the work once and let it move upwards. Nobody should retype what somebody below them has already recorded.

Anyone who has to justify a decision afterwards can use it. What differs is the tier you sit in, because a care assistant, a registered nurse, a registered manager and a director are answerable to different people for different things.

What would I struggle to evidence if somebody asked tomorrow?

What needs attention today, what has been delegated, what remains your accountability, what is overdue, and which improvements are actually holding.

Which service needs me today?

Several services at once: overdue actions, issues repeating across sites, which manager needs support, and whether something is isolated or a pattern.

Director or nominated individual

Can we show we exercised oversight?

Registered services, governance exceptions, regulatory readiness, safeguarding themes, audit trends, complaints, incidents and the evidence behind each.

Founder or owner

Is the system working?

Commercial position, operational performance, governance risk, escalations, workforce pressure and the decisions that need your authority. Not every case detail.

Who decided, on what, and when?

Every consequential output carries who assembled it, the evidence used, who reviewed it, the decision owner, the date and the next review.

Four tiers

Same evidence trail. Different accountability.

The tier is set by what you personally have to answer for, not by your job title or the size of your employer.

Tier one — Frontline

Care and support staff, healthcare assistants, team leaders and deputies.

Record once, properly, at the point it happens. Know what has to be escalated and be able to show that it was. Usually bought by the employer as seats.

Tier two — Registered professional

Nurses, doctors and GPs, paramedics, social workers, associate practitioners, allied health professionals, chaplains and ministers.

You hold your own registration and answer to your own regulator. This tier keeps your decisions, your reasoning and your supporting evidence in a form you can produce for revalidation, supervision, a coroner or a panel. Bought by the individual, and it travels with you between employers.

Tier three — Registered manager

Registered and deputy managers.

The accountability spine. What needs attention today, what has been delegated, what remains yours whatever you delegated, and what you would struggle to evidence if somebody asked tomorrow.

Tier four — Oversight

Regional and area managers, regional directors, directors, nominated individuals, owners and founders.

Above registered manager the question changes from “is this case handled” to “is the system working”. Patterns across services, governance exceptions, regulatory readiness and the decisions that need your authority.

A tier two professional inside a tier three service does not pay twice, and does not lose their own record when they move employer.

Boundaries

What the reasoning layer does, and where it stops

It organises information, finds gaps and contradictions, connects related material, routes work, prepares drafts, surfaces patterns and sets out the questions a person should consider.

It does not make safeguarding decisions, determine guilt, replace professional judgement, approve its own work, authorise expenditure or exercise statutory functions.

An observation is not automatically a finding. Overwatch raises what may need attention; a named person decides what it means.

The founder rule

Never feel better informed than the evidence justifies

Where information is missing, the system says it is missing. Where a conclusion is inference, it is labelled inference. Where a human decision is required, the system stops and waits for a person. Comfortable-looking dashboards that quietly fill their own gaps are the risk we built this to remove.
