Good review connects what is written with what people can actually observe, experience and evidence.

Look at the issue from more than one evidential position.
Compare records, accounts, observations, timing and the service environment. What a person could realistically know or see matters.
“What does this source show — and what can it not show?”

The current problem is an outcome. It is not automatically the explanation.
Reconstruct what came before: staffing, handovers, previous findings, actions, incidents, decisions and changes in the service.
“The end state tells us something. It does not tell us everything.”

Return to the human before deciding what the evidence means.
Technology can surface gaps and organise evidence. People provide context, challenge assumptions and remain accountable for consequential judgement and what happens next.
